ANIMAL AMBASSADOR TEAM ROLE

This application is for the animal ambassador team role at the OMHS. We ask that you please click here to review the volunteer post prior to completing the application.

Are you 18 years of age or older?

Do you wish to volunteer because you are required to for court ordered community service hours?

Have you had a tetanus booster in the last 10 years?

Have you had a pre-exposure rabies vaccine? (note: not required to volunteer)

Are you willing to provide references?

Are you willing to obtain a police background check?

Are you willing to complete Accessibility for Ontarians with Disabilities Act (AODA) training?

Do you have access to reliable transportation for yourself and your dog to get to/from the shelter and/or special events in Oakville and Milton?

What days and times are you available to volunteer?

Is your dog spayed/neutered?

Does your dog have a history of biting?

Is your dog house-trained?

Does your dog jump up on people or vocalize when they get excited or meet new people?

If your dog licensed?

Is your dog up to date on their rabies vaccination?

Are you willing to provide proof of rabies vaccination (i.e., rabies certificate)?

Is your dog up to date on their DHPP or DAPP vaccination?

Are you willing to schedule an assessment for your ambassador team (you and your dog) to see if you are the right fit for OMHS?
