P.A.T.C.H. – Recognize a Kind Child
Nominate a kind child for the P.A.T.C.H. Challenge.
Do you know a child who loves animals and has done great things to help them? Each year, OMHS recognizes a child who helped an animal(s). If your child has shown an act of kindness to an animal, we would like to hear from you. Complete the form below to nominate a kind child in the P.A.T.C.H. Challenge.
All nominees will receive a letter of acknowledgement and the winner will be drawn at random. This presentation is made possible thanks to the generosity of Mr. Frank Apa, President of Lexus of Oakville and founder of the P.A.T.C.H. Challenge.
To nominate a child, please fill out the form below.
